Legal Online Appointment Policy
How online appointment requests work.
Effective November 1, 2026
Our online form makes it easy to request a visit at our Encino or Palmdale office. Here's how it works.
A request, not a confirmed booking
Submitting the form sends a request to our front desk. Your appointment is confirmed only when our team contacts you by phone, text or email with a date and time.
When we'll reply
We reply during office hours, usually within an hour and no later than the next business day. Requests sent on evenings, weekends or holidays are answered the next business day.
- Encino: Monday to Friday, 8:30 AM to 5:00 PM
- Palmdale: Monday to Friday, 8:30 AM to 5:30 PM
What to include
Your name, phone number, email, preferred office, and the days and times that work for you. Please do not include medical history, symptoms or other health details. The form is for scheduling only, and we'll collect health information securely at your visit.
Not for emergencies
If you have a medical emergency, call 911. For a dental emergency, call us directly: Encino (818) 905-7787 · Palmdale (661) 224-9333.
How we use your request
We use what you submit only to schedule and contact you about your appointment, as described in our Privacy Policy and HIPAA Notice of Privacy Practices. Text messages are sent only if you opt in. See our SMS Terms.
New patients
Please arrive a little early, bring a photo ID and insurance card, and complete our new patient forms before your visit if you can.
Changes and cancellations
To change a confirmed appointment, call the office at least 48 hours ahead. See our Cancellation & No-Show Policy.
