This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Farzad Dental is required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
Treatment. We use your health information to provide and coordinate your dental care, including sharing it with other providers involved in your treatment.
Payment. We may use and disclose your information to bill and obtain payment from you, an insurance company, or a third party.
Health care operations. We may use your information for operations such as quality assessment, staff training, and appointment reminders.
We may contact you by phone, text message, or email to provide appointment reminders, results, or information about treatment options and services. You may ask us to communicate with you in a specific way or at a specific location.
You have the right to: inspect and obtain a copy of your health information; request an amendment to your record; request an accounting of certain disclosures; request restrictions on certain uses and disclosures; request confidential communications; and obtain a paper copy of this Notice upon request.
Other uses and disclosures not described in this Notice will be made only with your written authorization, which you may revoke at any time in writing.
If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
